Added recent changes to README

- Code16K fixes: remove extended ASCII latch, pad character
- Restructuring to have common code centralized:
  - Code128 routines for Codablock&Code16K
  - output.c has common routines from raster.c and symbologies.
- make internal routines invisible by a static function definition.
