UPGRADING: Add "no-op" messages for all versions

It's better to be explicit about version upgrades where no changes were
needed, so this patch updates the UPGRADING.md file to include them.

Note it includes the upcoming 0.06 version, for which no
backwards-incompatible change is expected.
